CHADIS (Child Health and Developmental Interactive System), a powerful internet-based screening tool scientifically proven to be among the best available. CHADIS is designed to help parents and health care providers take care of children better by offering high-quality screening questionnaires to help address parents' concerns about their child's development. CHADIS questionnaires and screening tools are accessed online and are completed in the comfort of your home prior to your child's appointment.

Studies have shown that screening is important for a variety of reasons; developmental problems such as autism or learning problems can be present without outward signs to parents; depression in a teenager can be fatal. Early diagnosis can shorten the time to recovery and improve the ultimate outcome.

CHADIS Screening Tools and Questionnaires Will Be Assigned For Each Check-Up

While the American Academy of Pediatrics and other child health agencies recommend screening, the time constraints on most office visits make it difficult. CHADIS screening tools and questionnaires will be assigned for each check-up. Your effort to complete the screening questionnaire(s) prior to your child's appointment will help us be better prepared for your concerns. To log into CHADIS, you will need to access your Patient Portal from our website or from our app on your cell phone. If you do not have access to your Patient Portal, please call our office and we will get you set up immediately. Please allow roughly 30 minutes to complete the questionnaire(s) once you have logged in.
